For referring providers

For physicians, nurse practitioners and allied health providers referring a patient for hormone or metabolic assessment.

Before You Submit

  • Confirm the patient consents to the referral and to being contacted directly.
  • Include recent bloodwork if available; it shortens the first appointment.
  • Care is virtual and province-wide; the patient does not need to be local.

Not covered by OHIP. Please make the patient aware that fees apply before referring.
